Section 1: INTRODUCTION
We ("Serum 29") are committed to protecting the privacy of client information and handling your personal information in a responsible manner in accordance with the Privacy Act 1988 (Cth), the Privacy Amendment (Enhancing Privacy Protection) Act 2012, the Australian Privacy Principles, and the relevant State and Territory privacy legislation. This legislation governs the ways in which we collect, use, disclose, store, secure, and dispose of your personal information. Section 2: COLLECTION
We collect information that is necessary and relevant to provide you with medical care, treatment and manage our clinic. This information may include, but is not limited to, your:
This information may be stored on our computer-based database and/or handwritten records. However, we may also need to collect information from other sources such as treating specialists, or other health care providers. We collect information in various ways, such as over the phone, in writing, in person in our clinic, or over the internet if you transact with us online. In an emergency situation, we may also need to collect information from your family and friends. We may be required by law to retain your personal records for certain periods of time.
Section 3: USE AND DISCLOSURE
We will treat your personal information as strictly private and confidential. We will only use or disclose it for purposes directly related to your care and treatment, or in ways that you would reasonably expect that we may use it for your ongoing care and treatment.
There are circumstances where we may be permitted or required by law to disclose your personal information to third parties. This includes but is not limited to Police, insurers, solicitors, government regulatory bodies, tribunals, courts of law, hospitals, and the electronic transfer of prescriptions.
We may disclose information about you to carry out activities on our behalf, such as a solicitor or debt collection agent. We impose security and confidentiality requirements on how they handle your persona information. Outside contractors are required to not use information about you for any purpose other than the activities we have contracted them to perform.
Section 4: DATA QUALITY AND SECURITY
We will take reasonable steps to ensure that your personal information is accurate, complete, up to date, and relevant. To enable us to do so, our staff may ask you to confirm that your personal information is correct when you attend an appointment. We request that you let us know if any of the information we hold about has recently changed, is incorrect, or out of date.

If you believe that the information we have about you is not accurate, complete, or up to date, we request that you contact us in writing (per details below).
Section 6: ACCESS
You are entitled to request access to your personal records. We request that you do so in writing and we will respond accordingly within a reasonable timeframe. Prior to the release of any personal information, we require signed consent from the Client. We may deny access to your personal records in certain circumstances as permitted by law, for example, if disclosure may cause or lead to a serious threat to your health and safety.

If you have a complaint regarding the privacy or handling of your personal information, we request that you contact us in writing at the E-mail address detailed below. Upon receipt of a written complaint, we will attempt to resolve it in accordance with current legislation and our in-house complaints handling policy.
If you are dissatisfied with our handling of a complaint or the outcome, you may make an application to the Australian Information Commissioner (AIC) or the Privacy Commissioner in your State or Territory.
Section 8: OVERSEAS TRANSFER OF DATA
We will not transfer your personal information to an overseas recipients or third-party unless we obtain your explicit written consent or are required to do so by law.

This policy is current from 1st November 2020 and is reviewed annually.
